WHEN A LOSS OCCURS IT HAS TO BE REPORTED TO THE OFFICE OR FIELD PERSON IMMEDIATELY AND A CLAIM FORM WILL BE FAXED OUT. THE CLAIM FORM AND PROOF OF LOSS MUST BE BACK TO THE OFFICE WITHIN TWO WEEKS OR THE CLAIM MAY BE DENIED.
